    A Service Agreement will then be sent outlining exact costs of the assessment process.

  • A two hour face to face assessment session at your home, school, work, or community including informal questions and standardised assessments. We tailor the assessment process to meet your needs and abilities.

    Born To Thrive Therapy provides:

    • Functional Capcity Assessments

    • NDIS eligibility assessments

    • Learning support recommendations for school.

  • If required, other information gathering may occur via phone, email, or reviewing previous reports to gain an in-depth understanding of the person across all of their environments and occupations.

  • Report writing between 2.5-6 hours, as agreed prior, to outline the findings of the assessment and the persons’ current functioning, and make recommendations for supports.

  • A 10-15 minute phone call to discuss needs, strengths, and concerns, to see if occupational therapy is a good fit and what intervention may be most appropriate.

    A Service Agreement will then be sent outlining exact costs of the intervention process.

  • A one hour session either face to face or via telehealth to gather background information, identify strengths and barriers, collaborate to create goals, and outline the intervention process.

    This is a tailored session to explore and understand how occupational therapy can help the person to achieve their goals.

    If the sessions are for a child, it is strongly encouraged that for the first intake session the child is not present, to openly discuss the child’s functional abilities. Children can be sensitive to information that may be perceived as weaknesses, we want to adopt a strengths based approach to identifying challenges and strengths collaboratively with the child in following sessions.

  • Intervention sessions provide evidence based strategies and activities to improve a certain skill, change the environment, or adapt a task to enable the person to achieve their goal of improved participation.

    Intervention sessions provided at Born To Thrive Therapy are tailored to meet the needs of individuals and can be:

    • 60 minutes (50 minutes face to face, 10 minutes note taking)

    • At the person’s home, daycare, school, or community environment (as agreed upon in the intake session and dependent on daycare/school policies).

    • Weekly, fortnightly, or monthly frequency.

    • Over a 10 week block across the school term.

  • How will we know when we achieve the goal?

    At Born To Thrive Therapy, we want to ensure our services are meaningful and goal orientated.

    Every 10 weeks, during the school holiday period, we use the scheduled intervention session to collaborate together review progress. This looks like:

    • Discussing strategies and activities we completed together.

    • Sharing observations of what the person has achieved outside of therapy, in their home, daycare, school, and community.

    • Identifying what progress has been made, what have we seen and heard that is different?

    • Celebrating wins the person has had and identify barriers to achieving their goal.

    • Discussing the progress and reviewing if we have achieved the goal.

    • If we have achieved the goal - what is next?

    • If we have not achieved the goal- what needs to continue or change?

    • Discussing strategies that can continue at home, daycare, school, or the community to support the person to achieve their goal.

    These discussions and changes to goals or intervention plans will be documented and sent to relevant people for reference.

  • If applicable, every 12 months, a report may be required to outline the individuals progress towards achieving their occupational therapy goals, or their functional abilities to participate in certain tasks or environments (e.g. school learning recommendations).

    Hours to complete requested reports will be outlined in writing prior to completion.
